    Mucocele de etmoide posterior simulando neurite óptica: relato de caso

    Mucoceles são formações císticas que podem ocorrer em variadas topografias na cavidade nasal, principalmente e na região do seio frontal e das células etmoidais, levando a uma clínicadecorrente de seu efeito compressivo local e apresenta a cirurgia como a principal forma de manejo terapêutico. O objetivo deste relato de caso é descrever o quadro clínico de um jovem do sexo masculino com uma mucocele em região de células etmoidais posteriores que abriu sua clínica de sintomas simulando os achados de uma neurite óptica, levando a apresentar uma clínica de perda visual do lado direito. Após seu tratamento inicial com corticoide não apresentar resultados clínicos, o exame de imagem foi capaz de identificar a descrição radiológica clássica de uma mucocele, transformando o diagnóstico e tratamento do paciente. Como conclusão, sugere-se que o caso apresentado seja uma referência no processo de construção de hipóteses diagnósticas dentro de um pensamento em neurite óptica, levantando sempre a necessidade de pensar em mucocele, e reiteramos que o tratamento cirúrgico é o de escolha no manejo desses pacientes.

    Ferramentas de e-learning para melhoria do aprendizado em medicina

    Introdução: As tecnologias de informação e comunicação (TIC) têm sido cada vez mais utilizadas no contexto da educação e vêm se tornando importantes ferramentas no auxílio do processo de ensino-aprendizagem na área da saúde. A Interactive Spaced Education (ISE) vem surgindo como método inovador para otimização do aprendizado em saúde. Objetivo: Investigar o uso da ISE como ferramenta de e-learning para melhoria do aprendizado do estudante de semiologia médica. Métodos: Estudo descritivo realizado com 98 estudantes regularmente matriculados no componente curricular “Iniciação ao Exame Clínico”, do curso de medicina. Os estudantes receberam componentes de ISE contendo itens referentes à semiologia cardiovascular e respiratória, duas vezes por semana, durante 13 semanas. Ao final das 13 semanas, os alunos receberam um questionário semi-estruturado online, contendo quesitos fechados utilizando escala de Likert, além de questões abertas, para avaliar a utilização da ferramenta ISE como adjuvante do aprendizado em semiologia médica. Resultados: A taxa de resposta ao questionário online foi de 67,4% (N=66). A idade média dos participantes foi de 20,9±2,2 anos, com predomínio do sexo masculino (54,5%). A ISE contribuiu para o aprendizado de 98,5% destes (N=65), sendo esta contribuição referente ao aprendizado teórico e prático para 59,1% dos indivíduos (N=37). A maioria dos participantes considerou boa a frequência de liberação semanal dos componentes ISE (N=29, 43,9%). Todos os estudantes indicariam a ISE para outros estudantes como forma de aprendizado. Conclusões: A ISE foi bem aceita como ferramenta de e-learning, importante para o aprendizado na percepção dos estudantes de semiologia médica.

    Response time in the emergency services. Systematic review

    The growth of the urban population raises concern about municipal public managers in the sense of providing emergency medical services (EMS) that are aligned with the needs of prehospital emergency medical care demanded by the population. The literature review aims at presenting the response time of emergency medical services in several parts of the world and discussing some factors that interfere in the result of this indicator such as GDP (Gross Domestic Product) percentage spent on health and life expectancy of countries. The study will also show that in some of the consulted articles, authors suggest to EMS recommendations for ecreasing the response time using simulations, heuristics and metaheuristics. Response time is a basic indicator of emergency medical services, in such a way that researchers use the descriptive statistics to evaluate this parameter. Europe and the USA outstand in the publication of studies that present this information. Some articles use stochastic and mathematical methods to suggest models that simulate scenarios of response time reduction and suggest such proposals to the local EMS. Countries in which the response time was identified have a high index of human development and life expectancy between 74.7 and 83.7 years

    Fibrin rich in leukocytes and platelets (L-PRF) and bisphosphonate-induced osteonecrosis: we need to advertise now

    The American Association of Oral and Maxillofacial Surgeons (AAOMS) reported in 2004 a new pathology that causes osteonecrosis in the Maxillomandibular complex. The condition was named Bisphosphonate-related osteonecrosis of the jaw (BRONJ), and later renamed by medication related to jaw osteonecrosis due to the appearance of severe alveolar bone loss in the Maxillomandibular complex resulting from the use of different drugs other than Bisphosphonates. Odontology, especially oral surgery, a field concerned with newbiotechnologies, has been introducing innovations in tissue engineering. Oro-maxillofacial reconstruction is of great interest to current oral and maxillofacial surgeons, as part of the search for strategies in bioengineering and biomaterials, a major promoter of bio-dental research in our times. In order to verify the scientific evidences to analyse the effectiveness of leukocyte and platelet rich fibrin (L-PRF) on bone neoformation inMaxillomandibular complex’s surgical procedures, a search for qualified articles was conducted in the Medline (PubMed), Embase and Cochrane Central Register of Controlled Trials databases. The works analysed were published from 2013 to 2018. This review concludes that the use of platelet rich fibrin (L-PRF) is significant in the complete healing of the lesion in short postoperative periods and in the resolution of the treatments, reducing the need for postoperative re-interventions, resulting in lower morbidity for the patients

    Development of new techniques for behavioral evaluation In animals submitted to facial nerve damage

    Three new methods for evaluating facial nerve injury were developed: Spontaneous ocular opening, vibrissae reflex and resistance to upper eyelid lifting. Nerve crush injury was performed by pressing the mice’s facial nerve with forceps for 30 seconds. Scores were given based on the comparison between the behavioral assessment of the injured and non-injured sides. Facial nerve injury is currently evaluated by observing the movement of the mice’s vibrissae and the ocular closure. In this study, three additional tests are proposed. Therefore, a wider evaluation can be accomplished where all the aspects of the facial mimicry can be assessed. The use of the 3 new methods described in this study, in addition to the 2 methods currently used, allows a complete behavioral assessment of facial nerve lesions as several behavioral aspects related to these injuries can be evaluated

    Combination sciatic nerve graft and fibroblastic growth factor 2 promotestissue regeneration for NF-200 and 5-HT in spinal cord injury

    Traumatic injury to the spinal cord results in a rapid and significant loss of function. One barrier to successful regeneration in the adult CNS is the diminished axonal growth capacity after maturation. Therefore, strategies that seek to promote the restoration of function to the chronically injured spinal cord have high therapeutic value. Neurotrophic factors and peripheral nerves are known to be good substrates for bridging the lesions associatedwith CNS trauma. The role of fibroblast growth factor-2, when added to the sciatic nerve, was examined following spinal cord injury in a rat. We evaluated whether FGF-2 added to a sciatic nerve graft placed in a gap promoted nerve recovery following a complete transection of the spinal cord and if it could enhance neuronal plasticity. Rats underwent a transection at the thoracic level, which was repaired with saline or a fragment of the sciatic nerve. In another group, FGF-2 was added immediately after thelesion. The effects of FGF-2 and the fragment of the sciatic nerve graft on neuronal plasticity were investigated at the epicenterof the injury using NF-200 and 5-HT immunoreactivity after 8 weeks.A high number of NF-200 and 5-HT immunoreactive fibers were observed in the treated groups with sciatic nerve graft in the presence or absence of FGF-2 when compared to the saline group. However, a small number of NF-200(p=0.03)and 5-HT fibers were observed in the epicenter of the graft when FGF-2 was added,when compared to the group that received sciatic nerve graft. These results indicate that sciatic nerve grafting favors the growth of fibers in the traumatized spinal cord, an effect that is slightly influenced by the addition of FGF-2 to the NF-200 and 5-HT immunoreactive fibers

    Effects of Mucuna pruriens (L.) supplementation on experimental models of parkinson's disease: a systematic review

    Introduction: Mucuna pruriens (L.)DC. (Mp) has been used in the treatment of numerous diseases in Indian Ayurvedic medicine, mainly for delaying the symptoms of Parkinson’s disease (PD), and has as its main component levodopa (L‑DOPA). The aim of this work is to systematically review the effects of Mp supplementation on experimental models of PD due to its neuroprotective and antioxidant properties. MATERIALS AND Methods: The search was conducted through PubMed, ScienceDirect, Cochrane Library, and Scientific Electronic Library Online databases, where a number of relevant articles were found. Results: Mp demonstrated significant positive responses in the experimental models of PD by improving motor deficits and by enhancing the activity of the antioxidant systems, reducing oxidative stress. In addition, it presented some advantages when compared to a conventional antiparkinsonian drug as it minimized the occurrence and severity of dyskinesias. Conclusion: Thus, considering that the use of herbal medicines decreases the likelihood of side effects associated with the pharmaceutical drugs, this work aims to summarize and evaluate the data available regarding the mechanism of action of Mp and the reported benefits of this plant as an alternative to improve the quality of life of individuals with PD

    Expansion and phenotypic changes of mouse bone marrow mesenchymal cells cultured with FGF-2 and facial nerve-conditioned medium

    Mesenchymal cells (MCs) exhibit great regenerative potential due to their intrinsic properties and ability to restore tissue function, either directly through transdifferentiation or indirectly through paracrine effects. This study aimed to evaluate morphometric and phenotypic changes in MCs grown with facial nerve-conditioned medium in the presence or absence of fibroblast growth factor 2 (FGF-2). For quantitative phenotypic analysis, the expression of GFAP, OX-42, MAP-2, β-tubulin III, NeuN, and NF-200 was analyzed by immunocytochemistry. Cells cultured with facial nerve-conditioned medium in the presence of FGF-2 expressed GFAP, OX-42, MAP-2, β-tubulin III, NeuN, and NF-200. On average, the area and perimeter of GFAP-positive cells were higher in the group cultured with facial nerve-conditioned medium compared to the group cultured with conditioned medium and FGF-2 (p=0.0001). This study demonstrated the plasticity of MCs for neuronal and glial lineages and opens up new research perspectives in cell therapy and trans.differentiation

    Astragaloside IV supplementation promotes a neuroprotective effect in experimental models of neurological disorders: a systematic review

    Background: Neurological disorders constitute a growing worldwide concern due to the progressive aging of the population and the risky behavior they represent. Herbal medicines have scientific relevance in the treatment of these pathologies. One of these substances, Astragaloside IV (AS-IV), is the main active compound present in the root of Astragalus membranaceus (Fisch.) Bge, a Chinese medicinal herb with neuroprotective properties. Objective: In the present study we performed a systematic review that sought to comprehend the neuroprotective effect presented by AS-IV in experimental models of neurological disorders. Methods: This study is a systematic review, where an electronic search in United States National Library of Medicine (PubMed), Science Direct, Cochrane Library, Scientific Electronic Library Online (SciELO), Scopus, Web of Science, Medline via Proquest and Periodicos Capes databases covering the years between 2007 and 2017, using “Astragaloside IV” and “Neurodegenerative diseases”; “Astragaloside IV” and “ Neurological disorders” as reference terms was made. Results: A total of 16 articles were identified, in which the efficacy of AS-IV was described in experimental models of Parkinson’s disease, Alzheimer’s disease, cerebral ischemia and autoimmune encephalomyelitis, by improving motor deficits and/or neurochemical activity, especially antioxidant systems, reducing inflammation and oxidative stress. Conclusion: The findings of the present study indicate that the administration of AS-IV can improve behavioral and neurochemical deficits largely due to its antioxidant, antiapoptotic and antiinflammatory properties, emerging as an alternative therapeutic approach for the treatment of neurological disorders

    Análise de desempenho sobre redes IEEE 802.3/PM-AH: protocolo multicilos para automação hospitalar

    Com a incorporação de conceitos da automação em ambientes hospitalares surge uma série de novos requisitos pertinentes a área médica. Dentre esses requisitos, um que merece destaque é a necessidade do estabelecimento de uma rede de comunicação segura e eficiente entre os elementos do ambiente hospitalar, visto que, os mesmos encontram-se de maneira distribuída. Nesse sentido, existe uma série de protocolos que podem ser utilizados no estabelecimento dessa rede, dentre os quais, um que merece destaque é o PM-AH (Protocolo Multiciclos para Automação Hospitalar) justamente por ser voltado a automatização de ambientes hospitalares tanto no que diz respeito ao cumprimento dos requisitos impostos nesse tipo de ambiente, como pelo fato de ser projetado para funcionar sobre a tecnologia Ethernet, padrão esse que é comumente utilizado pela rede de dados dos hospitais. Em decorrência disso, o presente trabalho aborda uma análise de desempenho comparativa entre redes PM-AH e puramente Ethernet visando atestar a eficiência do primeiro no que diz respeito ao cumprimento dos requisitos impostos pela automação hospitalar

    Quality evaluation under the perception of satisfaction of the users of different age groups in a Public Hospital in Natal - Brazil

    The present article endeavors to describe the levels of satisfaction of users of medical services at the University Hospital Onofre Lopes (UHOL) in different age groups and to study the association between users' satisfaction and the different types of services provided by this public health institution. The study is based on the Annual Satisfaction Survey conducted by the UHOL Ombudsman's office, which evaluates this information for health policy subsidy in this hospital. A total of 4,290 users were consulted between 2015 and 2017 divided into three age groups: adolescents, adults and the elderly. The dependent variable was patient satisfaction after medical consultation at the hospital, evaluated on a four-level Likert scale. The independent variables were Gender of respondents (G); Comfort at the reception hall (CRH); Hygiene, cleanliness and organization of the hospital (HCO); Comfort of facilities at the medical care area (CFMC); Reception Service (RS); Health team attendance (HTA); Attendance waiting time (AWT); Education Level (EL), Age (A) and Evaluation of General Satisfaction on Hospital Services (GS). The multiple linear regression and logistic regression analysis was completed, respectively, for the data in general and separately for each age bracket. Very few participants declared to be dissatisfied or very dissatisfied (4%). In the crude model, general satisfaction was associated with CRH and HCO in the adolescent group, CFMC and RS in the CRH, HCO, RS and HTA groups in the elderly group. Adolescents showed greater dissatisfaction with hospital services because they presented specific demands concomitant with their age group

    Control of payment of suppliers in a public hospital: case study of Onofre Lopes University Hospital – Brazil

    Throughout the last decades, the discussion about the quality in the health service has gained global importance, mainly due to the difficulty in reaching a thorough measurement for the performance of health services based on objective criteria. Thus, the purpose of the present study is to evaluate through Control Charts whether the supplier payment process in a public hospital is being completed within the time frames specified by internal corporate administrative resolutions. Initially followed the Montgomery methodology for non-self-correlated phenomena from which the application of individual control charts and moving range was utilized. Following the charts analysis, it became necessary to verify the conformity of the data in relation to the distribution. Hence, the Kolmogorov-Smirnov and Chi-Square tests were performed, in which it was verified that the Weibull distribution is better suited to the data, and control charts were applied for the Weibull distribution and Capability study. The results of the applications indicated variability in the process, which can be minimized by identifying the bottlenecks inherent to the formalities in the payment process; evaluating the time a given invoice remains with the purchasing coordinator; defining a timeframe necessary to comply with the process verification before validating the invoice and send it to the contract unity; determining the specific time requirements for each stage of the process as to optimize the flow; and establishing key performance indicators in order to attain an in depth understanding of the internal procedures

    Minnesota innovation survey at the Brazilian Hospital services company

    Promoting innovation is bringing to the field of organizations (public or private) a strategic bias recommended for decision-making. Knowing their determinants factors is one of the essential tasks that managers face due to the high level of complexity involved. The purpose of this study was to present, through the Minnesota Innovation Survey (MIS) method developed by the Minnesota Innovation Research Program (MIRP) of the University of Minnesota in the United States. The characteristics favorable to the innovation environment in University Hospitals agreed to the Brazilian Company of Hospital Services (EBSERH), considered one of the largest public company of public character in health services in Brazil. Seventeen hospitals (43.59%) from a total of 39 branches were surveyed in all regions of the country, through a structured electronic questionnaire containing 83 questions related to the 29 dimensions directed to the development of health innovations. With this, it was sought to determine the main variables related to change in this context. As a result, the characterization of eleven dimensions favorable to innovation in these hospital organizations was verified, with a strong correlation between them, determinants for an innovative environment in public health policies. In terms of contributions, the research developed an essential method for management, control and innovation of health care service providers, which can be used in a wide range of contexts, highlighting the main factors favorable to organizational environments focused on innovative processes, creating a theoretical basis pleasant practice for new research to be carried out using the methodology used

    Analysis of performance indicators applied in a university hospital conveniated to the Brazilian Hospital Services Company

    Introduction: In Brazil, university hospitals have fundamental importance in the evolution of the Unified Health System (SUS) and in several areas that favor the improvement of the health conditions of the population. Thus, this article aims to analyze performance indicators of the Onofre LopesUniversitary Hospital, located in the northeast region of the country, before and after joining the management contract with the Brazilian Hospital Services Company (EBSERH). Methods: In order to perform this study, the authors selected performance indicators in hospital organizations and analyzed the comparativeof the bienniums 2011-2012 and 2014-2015, based on the performance index matrix. The 32 indicators are organized in terms of structure and results, and in the categories: installations, human resources, revenue, production, productivity, quality, costs and teaching/research/extension. Results: The results show better performance of the indicators in the biennium 2014-2015. The authors attribute this evolution of the performance indexes to the expansion and reorganization of the work team, to the increase in revenue, to structural reforms and to the provision of new services to the population. Conclusion: The small number of studies that relate the results of the indicators in a periodic comparison of the same hospital is noticeable. Therefore, is essential the monitoring, analysis and dissemination of these indicators
