    Application of QFD methodology as a planning tool for quality management on a clinical engineering sector at HUOL / UFRN

    In a complex hospital environment, where in many cases the demands for a quality in Clinical Engineering are high and resources are low, it is important to apply tools that can assist at improving services provided by clinical engineering’s professionals. Thus, this paper aims to apply the QFD methodology as an aid tool in planning the quality of service provided by the clinical engineering team of Onofre Lopes University Hospital, with the intention of proposing improvements to be included in future planning. Initially, this research aims to discover the main needs of healthcare professionals that use services provided by the clinical engineering team, by reviewing articles on quality at hospitals environments. The target sectors of this research were chosen using as criteria the highest number of work orders (OS) generated and criticality of equipment. It is also object of study to analyze the level of satisfaction and importance that the employees of the studied sectors attribute to the clinical engineering sector. This study was conducted through semi-structured interviews, which were granted by the heads and coordinators of each sector. After completing the previous step, the quality house was built starting with translating users' needs into design requirements, and then correlating existing design requirements with finding out which design requirement is of highest relevance to meet customer needs. In the end, it will be possible to generate data that can be interpreted efficiently by the clinical engineering team, thus identify which points are working as expected and which need to be analyzed more closely in future strategic planning

    O hospital é uma organização que possui características próprias que o diferenciam de outras instituições. Ele se caracteriza como uma empresa de prestação de serviços de finalidade social, com uma grande complexidade operacional, em função da diversidade dos serviços prestados. Essas organizações são dependentes da tecnologia para que exerçam o papel de assistência à sociedade. A tecnologia é onerosa: cabe as instituições supracitadas adotar ferramentas de gestão para o controle destas onerações. Essa dissertação segue nesse sentido e propõe a implantação de uma central de equipamentos (CENTROMED) para a gestão dos equipamentos médico hospitalares (EMH) no Hospital Universitário Onofre Lopes (HUOL). Para alcançar esse objetivo serão necessárias modelagens de processos, determinação de indicadores chave de performance e definição do arranjo físico. Além disso, treinamento para a equipe que compõe essa central no que tange as melhores práticas de uso se faz necessário para a utilização adequada dos equipamentos. O percurso metodológico adotado neste trabalho consiste na identificação de demandas junto à equipe de engenharia clínica do HUOL, definição do processo de prestação do serviço de equipamentos além da definição dos itens de fornecimento que serão gerenciados. Os resultados preliminares dessa pesquisa sinalizaram que a centralização da gestão dos EMH’s contribui para a otimização dos custos e disponibilidade destes itens representando dessa forma uma poderosa ferramenta de gestão que colabora para o atendimento das equipes assistenciais.

    Control of payment of suppliers in a public hospital: case study of Onofre Lopes University Hospital – Brazil

    Throughout the last decades, the discussion about the quality in the health service has gained global importance, mainly due to the difficulty in reaching a thorough measurement for the performance of health services based on objective criteria. Thus, the purpose of the present study is to evaluate through Control Charts whether the supplier payment process in a public hospital is being completed within the time frames specified by internal corporate administrative resolutions. Initially followed the Montgomery methodology for non-self-correlated phenomena from which the application of individual control charts and moving range was utilized. Following the charts analysis, it became necessary to verify the conformity of the data in relation to the distribution. Hence, the Kolmogorov-Smirnov and Chi-Square tests were performed, in which it was verified that the Weibull distribution is better suited to the data, and control charts were applied for the Weibull distribution and Capability study. The results of the applications indicated variability in the process, which can be minimized by identifying the bottlenecks inherent to the formalities in the payment process; evaluating the time a given invoice remains with the purchasing coordinator; defining a timeframe necessary to comply with the process verification before validating the invoice and send it to the contract unity; determining the specific time requirements for each stage of the process as to optimize the flow; and establishing key performance indicators in order to attain an in depth understanding of the internal procedures

    Fibrin rich in leukocytes and platelets (L-PRF) and bisphosphonate-induced osteonecrosis: we need to advertise now

    The American Association of Oral and Maxillofacial Surgeons (AAOMS) reported in 2004 a new pathology that causes osteonecrosis in the Maxillomandibular complex. The condition was named Bisphosphonate-related osteonecrosis of the jaw (BRONJ), and later renamed by medication related to jaw osteonecrosis due to the appearance of severe alveolar bone loss in the Maxillomandibular complex resulting from the use of different drugs other than Bisphosphonates. Odontology, especially oral surgery, a field concerned with newbiotechnologies, has been introducing innovations in tissue engineering. Oro-maxillofacial reconstruction is of great interest to current oral and maxillofacial surgeons, as part of the search for strategies in bioengineering and biomaterials, a major promoter of bio-dental research in our times. In order to verify the scientific evidences to analyse the effectiveness of leukocyte and platelet rich fibrin (L-PRF) on bone neoformation inMaxillomandibular complex’s surgical procedures, a search for qualified articles was conducted in the Medline (PubMed), Embase and Cochrane Central Register of Controlled Trials databases. The works analysed were published from 2013 to 2018. This review concludes that the use of platelet rich fibrin (L-PRF) is significant in the complete healing of the lesion in short postoperative periods and in the resolution of the treatments, reducing the need for postoperative re-interventions, resulting in lower morbidity for the patients

    Minnesota innovation survey at the Brazilian Hospital services company

    Promoting innovation is bringing to the field of organizations (public or private) a strategic bias recommended for decision-making. Knowing their determinants factors is one of the essential tasks that managers face due to the high level of complexity involved. The purpose of this study was to present, through the Minnesota Innovation Survey (MIS) method developed by the Minnesota Innovation Research Program (MIRP) of the University of Minnesota in the United States. The characteristics favorable to the innovation environment in University Hospitals agreed to the Brazilian Company of Hospital Services (EBSERH), considered one of the largest public company of public character in health services in Brazil. Seventeen hospitals (43.59%) from a total of 39 branches were surveyed in all regions of the country, through a structured electronic questionnaire containing 83 questions related to the 29 dimensions directed to the development of health innovations. With this, it was sought to determine the main variables related to change in this context. As a result, the characterization of eleven dimensions favorable to innovation in these hospital organizations was verified, with a strong correlation between them, determinants for an innovative environment in public health policies. In terms of contributions, the research developed an essential method for management, control and innovation of health care service providers, which can be used in a wide range of contexts, highlighting the main factors favorable to organizational environments focused on innovative processes, creating a theoretical basis pleasant practice for new research to be carried out using the methodology used

    Insecurity is a public problem and is commonly present in the population's daily lives. It is notorious that many people seek alternatives to minimize or avoid situations that cause risks to them or their loved ones. Technology proves to be an efficient tool in this scenario, since it assists in diverse situations and in a fast and practical way. The present work aims to ally the technology with the problematic of the lack of security, through the creation of a mobile application, with the purpose of providing the end user with a proposal of help in adverse situations. The application can be used by anyone who has a mobile phone with an Android or IOS operating system. The ultimate goal of the application is to provide clients with a useful alternative so that they can mitigate the problems of personal insecurity on their return home, relying on the support of their family members or contracted services, such as private or street security. For the implementation of the project, tests are in progress, ensuring that all particularities are examined, before feasibility and final disclosure of the application

    Proposta para implantação de centrais de equipamentos médicos nos hospitais públicos estaduais

    O Sistema Único de Saúde (SUS) abrange Estabelecimentos Assistenciais de Saúde (EAS) com diferentes níveis de complexidade, demandando diversos equipamentos médicos. A Secretaria de Estado de Saúde Pública do Rio Grande do Norte (SESAP/RN) desempenha papel crucial na coordenação da Política Estadual de Saúde, aplicando os princípios do SUS e responsabilizando-se por estratégias de gerenciamento de equipamentos médico-hospitalares em 21 hospitais. A ausência de um sistema eficiente para essa gestão pode resultar em consequências adversas, como a indisponibilidade ou má manutenção de equipamentos essenciais, além de desperdícios financeiros. Este trabalho tem como objetivo desenvolver uma proposta de implantação de Centrais de Equipamentos médicos nas unidades hospitalares pertencentes à rede SESAP/RN, fundamentada na urgência de aprimorar o desempenho operacional, maximizar a gestão de recursos financeiros, manter a disponibilidade de aparelhos essenciais e proporcionar atendimento de excelência e com segurança aos pacientes. Por meio de um procedimento exploratório e quantitativo-qualitativo, foi realizada a coleta de dados acerca do organograma e processo de trabalho desenvolvidos durante as etapas do ciclo de vida de um Equipamento Médico Hospitalar (EMH) na SESAP/RN. Bem como, envolvendo as informações acerca das unidades que já possuem CEQUIPs. Então, baseada na metodologia de estudo de caso, foi proposto um novo processo de trabalho, levando em consideração o ambiente adequado e profissionais habilitados a fim de estruturar e implantar Centrais de Equipamentos nas unidades hospitalares da rede SESAP/RN. Com isso, foi possível a elaboração de um novo modelo de processo de trabalho centralizando os serviços relacionados a algumas etapas do ciclo de vida dos EMHs, serviços de manutenção, controle quanto aos contratos vigentes, acompanhamento de cessão dos equipamentos e a garantia do gerenciamento do parque tecnológico de uma unidade hospitalar da rede estadual de saúde.

    Quality evaluation under the perception of satisfaction of the users of different age groups in a Public Hospital in Natal - Brazil

    The present article endeavors to describe the levels of satisfaction of users of medical services at the University Hospital Onofre Lopes (UHOL) in different age groups and to study the association between users' satisfaction and the different types of services provided by this public health institution. The study is based on the Annual Satisfaction Survey conducted by the UHOL Ombudsman's office, which evaluates this information for health policy subsidy in this hospital. A total of 4,290 users were consulted between 2015 and 2017 divided into three age groups: adolescents, adults and the elderly. The dependent variable was patient satisfaction after medical consultation at the hospital, evaluated on a four-level Likert scale. The independent variables were Gender of respondents (G); Comfort at the reception hall (CRH); Hygiene, cleanliness and organization of the hospital (HCO); Comfort of facilities at the medical care area (CFMC); Reception Service (RS); Health team attendance (HTA); Attendance waiting time (AWT); Education Level (EL), Age (A) and Evaluation of General Satisfaction on Hospital Services (GS). The multiple linear regression and logistic regression analysis was completed, respectively, for the data in general and separately for each age bracket. Very few participants declared to be dissatisfied or very dissatisfied (4%). In the crude model, general satisfaction was associated with CRH and HCO in the adolescent group, CFMC and RS in the CRH, HCO, RS and HTA groups in the elderly group. Adolescents showed greater dissatisfaction with hospital services because they presented specific demands concomitant with their age group

    Response time in the emergency services. Systematic review

    The growth of the urban population raises concern about municipal public managers in the sense of providing emergency medical services (EMS) that are aligned with the needs of prehospital emergency medical care demanded by the population. The literature review aims at presenting the response time of emergency medical services in several parts of the world and discussing some factors that interfere in the result of this indicator such as GDP (Gross Domestic Product) percentage spent on health and life expectancy of countries. The study will also show that in some of the consulted articles, authors suggest to EMS recommendations for ecreasing the response time using simulations, heuristics and metaheuristics. Response time is a basic indicator of emergency medical services, in such a way that researchers use the descriptive statistics to evaluate this parameter. Europe and the USA outstand in the publication of studies that present this information. Some articles use stochastic and mathematical methods to suggest models that simulate scenarios of response time reduction and suggest such proposals to the local EMS. Countries in which the response time was identified have a high index of human development and life expectancy between 74.7 and 83.7 years
